Case Management That Starts With Listening

At most organizations, case management means filling out forms and getting a referral. At Food & Care Coalition, it means sitting down with someone who takes the time to understand your situation — your history, your goals, and the specific barriers standing between you and stability.

Our case managers coordinate across every service we offer — meals, housing, dental care, mental health support, and education — and connect you with partner agencies when you need something beyond what we offer on-site. The goal isn't to manage your case. It's to help you move forward.

Two Ways We Serve

Drop-In & Resident Case Management

Drop-In Support

Any patron receiving day services at Food & Care Coalition can connect with a case manager during regular business hours — no appointment required. Case managers help with immediate needs: identifying resources, navigating systems, and taking the first step toward a longer-term plan.

Resident Case Management

All 38 residents in our transitional and permanent housing programs receive scheduled, ongoing case management. Residents meet regularly with a dedicated case manager to set concrete goals, track progress, and access the full range of Coalition and partner services on their path to independent living.

Walk into Food & Care Coalition during business hours and let staff know you'd like to speak with a case manager. No referral or appointment needed. Just show up.

Your case manager sits down with you to understand your goals — housing, employment, health care, or whatever matters most. Together you build a realistic, judgment-free plan with clear next steps.

Case management doesn't end at one meeting. Your case manager stays alongside you — connecting you to services, checking in on progress, and adapting the plan as your situation changes.
